Free Templates for Clinicians

These templates are provided as a courtesy to the psychotherapy community. You may use, and alter them as you see fit. I only ask that you please keep my name in the bottom footer.

The documents and pull-down menu format are copyrighted. They are not for sale or distribution

I am in the process of creating a comprehensive therapist software that will incorporate both of these progress notes documents as well as many other templates and useful features. Look for Therapist Office Manager (TOM) in the future!

Considering Becoming a Clinician?

Psychotherapy is such a vast field, that I think it is oftentimes difficult to find good information about the basic meaning of the various terms related to it. Many people find themselves overwhelmed by the multitude of academic degrees, professional licenses and philosophical approaches associated with the practice of psychotherapy. But without an initial framework to address the topic, it can be challenging to determine what questions they may even want to ask. I have created a very brief overview of the field of psychotherapy to help address that dilemma for potential clients, but it also should be helpful to potential clinicians.

Summary of LPC/LPCC Licensure Requirements in MN

The LPC and LPCC licensure process in the state of Minnesota can be dizzyingly confusing. As someone who has successfully navigated this process, I feel obliged to make available to others what information I had learned along the way. I cannot provide a guarantee, of course, that the information below is current or accurate. For that you must view the definitive (but less clearly stated) information provided by the Minnesota Board of Behavioral Health (BBHT). If you have a specific question that you can't find the answer to in my materials or on their website, this is a link to the contact information for the Board Staff.
